Australia

Instructions
If you are outside Australia and your passport is lost or stolen you must immediately either report your passpost lost or stolen or report the loss to the nearest Australian diplomatic or consular mission.

If you are replacing your passport, you will need to:
  • Complete an overseas passport application form,
  • Provide details of the lost/stolen passport,
  • Attend an interview,
  • Pay the current application fee,
  • Pay the appropriate lost/stolen passport fee
If not replacing your passport you should complete a Lost or Stolen Australian passport form and send it to the nearest Australian diplomatic or consular mission.

Belgium

Instructions
  • Call Doc-Stop to cancel your passport and protect yourself against any fraudulent use of your identity: +32 2 518 2123
  • Notify the local police about the loss/theft immediately. Ask for a statement confirming the loss/theft.
  • If you need a passport to return to Belgium, contact the Belgian embassy or consulate in the country you are visiting. If there is none, contact the embassy or consulate of another EU country.
  • If you have been unable to notify the local police of the loss/theft while you were abroad, do so with your police station as soon as you return to Belgium.

Brazil

Previous passports which have been lost, damaged or stolen:
  • Lost or damaged passports: the applicant must sign in the presence of a consular agent a declaration explaining how the document was lost or damaged and declaring this to be the truth.
  • Stolen passports: the applicant must bring in the original police report stating that the passport was stolen.
  • Whenever a passport is lost, damaged or stolen, the applicant must submit proof of Brazilian citizinship - a Brazilian birth or marriage certificate, a Brazilian identification card, a Brazilian military document, and/or any other documents deemed necessary by the Consular authority. Request of service (for getting a number to be assisted): 9AM to 11AM. After 11AM only people with a service request number will be assisted at the Consulate. Canada

Instructions
Inform the local police, and get a police report or case number (if possible), then contact Passport Canada at 1-800-567-6868 or one of our offices to report your passport lost/stolen. Once it is reported lost, apply to Passport Canada for a new Passport. The holder must complete both the full application form and include form PPTC 203 (Declaration concerning a lost, stolen, inaccessible, damaged or found Canadian travel document).

France

Instructions
In the event of loss or theft of documents, it is imperative to file a report at the closest police station and ask for a copy so you may obtain replacement documents at the Consulate.
The Consulate may issue a 'laissez-passer' allowing you to return to France or country of origin (valid only for certain countries) or an emergency passport, valid for 1 year. In both cases you must come to the Consulate with the following documents:
  • Proof of identity and French nationality
  • 2 passport-sized photos, which must be recent, centered and full-faced, without head covering, on a white background
  • Airline tickets or electronic ticket
  • Police report on the loss or theft of documents
For a 'laissez-passer', a French witness in possession of a passport or national ID card may be acceptable as proof of French nationality.
The processing time for an emergency passport is several days.

Please note that the emergency service offers assistance only to German citizins needing immediate assistance or if you are calling on behalf of a German citizen who requires immediate assistance. It does not have access to files and can neither provide information on the status of passport or visa applications not answer general inquiries.
In case of an emergency involving a German citizen please call +1-202-298-4000. For lost passports citizens should call the Los Angeles office, who will advise as to whether the San Francisco office will need to step in.

Ireland

Instructions
From 17 January 2011, adult applicants in this category are required to submit the following additional evidence in support of their passport application:
A completed and suitable witnessed passport application form and four photographs, (two of which must have the application form number written on the reverse and be sined by an approved witness)
The Passport Service reserved the right to contact witnesses as required. Passport applications should therefore note that where it is not possible to make contact with those witnessing applicants, then it is likely that the application will be rejected.
And original police report for your lost or stolen passport or section 6 witnessed, signed, and dated by a police officer
The long form birth certificate of the applicant and a civil marriage certificate, if appropriate, and evidence of entitlement to Irish citizenship, where applicable;
Additional form of photo-identification; e.g. certified copy of drivers license; work ID; student card; social club membership; passport from another country etc.;
Documentary evidence to show use of name, e.g. payslips; records of unemployment or disability payment; bank statement; college registration etc,; and
Evidence of residence at the application address, e.g. utility bills; official correspondence from public or private sector etc.
The above-mentioned should be submitted in original format.
In addition, applicants may also be required to provide further information and/or attend for interview at one of the Department's offices.

It should be noted that it will not be possible to expedite any application in these categories and processing time will be 12-16 weeks.

Italy

Instructions
If at any point an Italian citizen is in an emergency situation (e.g. a tourist in transit who must leave and has lost or been robbed of his passport) and there is not enough time to confuct the necessary process to issue a new passport, the competent consular representative may issue a temporary travel document (also called ETD - Emergency Travel Document) that is valid only for the return journey to Italy, the country of permanent residence abroad, or in exceptional cases, to a different destination.
REQUIRED DOCUMENTS TO OBTAIN AN E.T.D. (EMERGENCY TRAVEL DOCUMENT) for tourists, that urgently need to return to Italy in the event of theft or loss of passport

The ETD is valid for only a few days and only for re-entry into Italy.
The obtain the ETD it is necessary to submit the necessary documentation in person, or at the Consulate.
  • Completed passport request form (the same as used to apply for an ordinary passport)
  • A valid identification document
  • 2 photographs of the interested party (identical, full-face color size 1.5"x1.5" on white background)
  • Consular report form for Lost/Stolen Passport
  • Plane tickets/itenerary/e-tickets
  • Payment for the cost of the document (currently $1.70 through June 30th, 2015)
The ETD can also be issued to citizens of EU member states after obtaining formal authorization by a consular of diplomatic representative of the Ministry of Foreign Affairs of the country of the applicant
If the applicant, rather than picking up the passport in person, would like to have his/her passport mailed, he/she must send a PREPAID, PREADDRESSED envelope EXCLUSIVELY from the US POST OFFICE - EXPRESS MAIL, with a signed authorization form allowing the Consulate to send the passport by mail.
We remind all applicants that, excluding the cost of the passport and any associated fees, filling out forms is FREE OF CHARGE. Do not trust unauthorized intermediaries who request payment. The information provided by applicants is private and reserved solely for internal use by Consular officials.

Netherlands

Instructions
In case of a lost or stolen passport you can apply for an Emergency Passport or, when you are traveling withing 3 business days, a Laissez Passer (LP) to return to the Netherlands (or your country of residence).

Please note that an emergency passport/LP is only issued after regular business hours in case of an extreme emergency (e.g. death in the family). In all other cases issuing will take place during regular opening hours.

Emergency passports and LP's are not accepted in all countries. Please make sure to clarify your full itenerary when applying, so it can be confirmed your destination country accepts the document.

When your regular passport is lost it is important to contact an office without delay. During regular business hours contact the nearest office for instructions. After business hours you can start the process by sending an email to the Netherlands Information Desk (info@dutchhelp.com) with the subject line: Lost passport.

In your message provide the following information:
  • Your full name
  • Date of birth
  • Place of birth
  • Place of issuance of the lost Dutch passport
  • Previous passport number (if known)
  • BSN-number (former sofi-number) (if known)
  • At what location and when you wish to apply for your emergency passport/LP
  • Copy of your itinerary with your intended travel date
  • Local contact information, daytime phone number(s), e-mail address and location (e.g. hotel name and address, local address)
After an appointment has been scheduled (contact the office of your choice directly), bring the following with you.
  • Passport application filled out and signed;
  • 'Verklaring vermissing reisdocument' form;
  • One recent passport picture (no more than 6 months old), according to the Dutch passport photo requirements. You are encouraged to have your picture taken by a recommended photographer. For LP issuing 2 passport pictures will be required;
  • Photocopy of passport (if available);
  • Other forms of ID (European ID card / Dutch drivers license) or photocopies of these ID's (if available);
  • Emergency passport fee (in local currency), in cash or Money Order. The missions in San Francisco and Los Angeles accept credit cards (MasterCard and Visa). The mission in Trinidad & Tobago only accepts cash;
  • Airline ticket for your return to the Netherlands;
  • Police report of the local police, stating your passport is missing or has been stolen;
  • Proof of emergency;
  • If in the US: If you entered the US on an ESTA Visa Waiver Program, please provide us with a copy of the document;
  • To have your new and old passport sent back to you: Please read the mailing instructions and submit a prepaid self-addressed label.

Switzerland

Instructions
The loss of theft of a Swiss ID document abroad must be reported as soon as possible to the local police as well as to the relevant Swiss representation.
A police report (declaration of loss) is also required to make it possible to prepare a new identity document, such as a temporary passport. Some countries may require such a document in order to leave their territory. It can also be useful when seeking reimbursement from an insurance company.
Swiss passports / identity cards which have been reported lost or stolen to a Swiss representation are entered in a Federak Office of Police (fedpol) file, and if subsequently recovered must not be used in an ID check or for travel, to avoid future difficulties. They are to be quickly returned to a Swiss authority for destruction. (Lost and Found Admin note: A hard copy of form, which needs to be completed, should be located with this spreadsheet)
