Brazil

Step 1: Fill out the passport form https://formulario-mre.serpro.gov.br/sci/pages/web/ui/#/passaporte-visto
Click on "passport". It is not necessary to fill in all fields. If any requested information is missing, check the box "unknown".

Step 2: After completing the travel document form, access the E-Consular platform: https://ec-saofrancisco.itamaraty.gov.br/
You must create a new registration. Registration can be done with your email address or CPF.
Click on "new service" > "Passaporte para maiores de 18 anos" ou "Passaporte para menores de 18" > "Start service". Fill in all the information requested by the system and digitally send images of the documents and USPS money order.
The E-Consular website is a preprocess platform of passport applications, which will only be finalized after the Consulate receives the documents by mail or in person appointment. Requests received without having been previously validated through the e-Consular system will not be processed within the established deadlines and may be returned without processing.

Step 3: After receiving validation from the E-Consular confirming that all documents are correct, access the link sent to your email address to schedule your service time.

If you wish to apply for the passport by mail, send all the documents listed above and the confirmation with QR code in an envelope from USPS – United States Postal Service. Don't forget to send a second prepaid envelope (with payment stamp) for return of the original documents and new passport. Accepted envelopes: USPS Priority Mail or USPS Priority Mail Express. Fill in your address in the "TO" field of the return envelope and save the tracking code for both envelopes.

For lost or stolen documents, please send an email to admin-francais.san-francisco-fslt@diplomatie.gouv.fr. Please do not come to the Consulate General without an appointment.

In the event of loss or theft of documents, it is imperative to file a report at the closest police station and ask for a copy so you may obtain replacement documents at the Consulate.

The Consulate may issue a 'laissez-passer' allowing you to return to France or country of origin (valid only for certain countries) or an emergency passport, valid for 1 year. In both cases you must come to the Consulate with the following documents:
  • Proof of identity and French nationality
  • 2 passport-sized photos, which must be recent, centered and full-faced, without head covering, on a white background.
  • Airline tickets or electronic ticket
  • Police report on the loss or theft of documents
For a 'laissez-passer,' a French witness in possession of a passport or national ID card may be acceptable as proof of French nationality.

Germany

Instructions
All German citizens should take an electronic copy (and share it with trusted others) of their travel document. If/When a passport is lost, the German citizen should contact the closest diplomatic office, likely the San Francisco Consulate General. The German citizen will need to apply for replacement/emergency/temporary (depending on the situation) documents in person, regardless of the situation.

Documents necessary for new documents are listed here:
- Completed application form
- two identical biometric recent passport photos (Please do not cut the photos yourself!)
- your birth certificate stating the exact place of birth (it is not sufficient if only the county is mentioned on the US birth certificate, a "proof of birth letter" issued by the hospital with the name of the town must then also be provided)
- if married, divorced or widowed: marriage certificate, if applicable divorce decree or death certificate of the spouse
- previous passport (data page with photo)
- Non-US-Citizens: valid residence permit in the US (US Resident Alien Card, US residence visa) and passport
- Information for holders of a greencard without expiration date
- US-Residents: Proof of Acquisition of US-Citizenship (e.g. US birth-certificate, Consular Report of Birth Abroad, US Certificate of Naturalization with retention permit) and US-passport
- if the last name in your previous passport is not the same as the last name you currently use, a name declaration might have to be done. Please consider the Information on Name Declaration
- Proof of current name usage (i.e. one of the following: German birth certificate, German marriage certificate or if married outside of Germany, certificate of current name usage issued by the registrar's office in Germany)
- if applicable, German Naturalization Certificate
- if applicable, US Naturalization Certificate with "Beibehaltungsgenehmigung"
- your US driver's license or an utility bill in your name as proof of residence
- if applicable, deregistration of your last place of residence in Germany ("Abmeldebescheinigung"), if it is entered in your last passport
- if you wish to have your doctoral degree entered in your passport, proof of residency and your old passport or German ID card with entry of your doctoral degree must be presented. If the doctoral degree is to be entered in the passport for the first time, your doctoral degree certificate must be submitted (the certificate should contain at least your first and last name, as well as your date of birth).
- Only for adult first time applicants: If one of your parents has had German citizenship at the time of your birth and if you therefore have obtained German citizenship by birth and not by naturalization, we need the following additional documents:
-- Passports of both of your parents (data page with a photo)
-- In case one parent is a US citizen without a passport: That parent's driver's license or State ID
-- Valid US residence title of the German parent (US Resident Alien Card or US visa)
-- Parents' marriage certificate
- Passport fee

In addition, some German missions in the United States require that a stamped envelope for mailing your documents already be submitted at the time of application. The missions abroad that have this requirement will inform you of this in your appointment booking confirmation.

Apply by paper application form for a replacement
- Fill out Section 6 of the application form and explain how your passport was lost, stolen or damaged.
- Get Section 6 witnessed and stamped.
If you do not intend to apply for a new passport and want to report a lost/stolen passport
- You will need to report your passport as lost/stolen to the Gardai and submit a Garda certificate of loss along with a letter signed by you, requesting that the passport be marked as lost/stolen.

Important details regarding lost/stolen passports
We report the details to Interpol.
The passport is no longer valid for travel.
If you report a passport as lost or stolen and subsequently locate it, do not travel on it.
We recommend you let the Gardai know to protect yourself from identity fraud.
We may restrict the validity of your next passport. Losing two or more passports may mean you do not get a full 10 year passport. You may get a new passport valid for somewhere between 1 and 10 years. This will depend on how many passports you have lost or had stolen over the last 10 years.
It may take longer to process the application for your next passport.

Issuance of an Israeli travel document (passport) following loss, theft or damage

This service enables Israeli citizens abroad to submit an application for a new travel document (passport) following loss, theft, damage or wearing out of the previous travel document.

Required Documents To Apply
- Identifying document with a photo (ID card or Israeli driver's license).
- Two recent passport photos (color against a white background, with full front view of face, measuring 5x5 cm).

The applicant must appear in person at the mission.

How to apply
The applicant must be over the age of 18.
Appearance in person is mandatory for the issuance of a new travel document abroad and for the issuance of a travel document due to loss / theft / damage.
Applicants below the age of 18 must appear in person, accompanied by their parents.
If the parents are married, it is enough for one parent to be present.

Please Note: The type of document issued will be determined by the consular representative at the mission.

Security instructions for arrival at the venue
Before your arrival, please follow the following instructions:
1. You must arrive with all your passports - if you have additional foreign citizenships, make sure you arrive with all your passports.
2. It is prohibited to arrive with weapons or self-defense means, including knives, batons, pepper spray, etc.
3. Do not bring electronic devices other than a mobile phone.
Visitors who arrive with computers/tablets will not be allowed entry.
4. No storage of items on site - avoid bringing non-essential equipment.
Also, do not arrive with valuable items and large sums of money.
5. Do not arrive with additional visitors who are not required for the purpose of the meeting.
6. If you have any disability or have undergone medical treatment, such as a pacemaker, platinums, etc., make sure you come with the documents confirming this.
7. There is a police officer in the lobby of the building, you must obey his instructions and act in accordance with his instructions.

Please note that this form of assistance may be unavailable during holiday closures, and in such cases it may be necessary to postpone the request for E.T.D. issuance until the next available workday.

Netherlands

Instructions
Follow the steps below if your Dutch passport or ID card has been lost or stolen abroad and you want to get back to the Netherlands.

1. Report the loss or theft to the local police
Report the loss or theft of your passport or ID card to the local police. It is advisable to ask the police for written proof that you have done this. You may need this proof later for insurance purposes or to show the local authorities.

2. Block your ID card's login feature
If your lost or stolen ID card was issued after 13 March 2021, someone could use it to log in with DigiD. So it is important to block the login feature. Contact the DigiD helpdesk to do this. This does not apply to passports.

3. Apply for an emergency travel document
Apply for an emergency travel document at the Dutch embassy or consulate-general. This is a temporary travel document.

Switzerland

Instructions
The loss of theft of a Swiss ID document abroad must be reported as soon as possible to the local police as well as to the relevant Swiss representation.
A police report (declaration of loss) is also required to make it possible to prepare a new identity document, such as a temporary passport. Some countries may require such a document in order to leave their territory. It can also be useful when seeking reimbursement from an insurance company.
Swiss passports / identity cards which have been reported lost or stolen to a Swiss representation are entered in a Federal Office of Police (fedpol) file, and if subsequently recovered must not be used in an ID check or for travel, to avoid future difficulties. They are to be quickly returned to a Swiss authority for destruction.
